It is very rare to see an Italian guy with a shirt over a t-shirt. The closest thing would be a shirt over a vest, but with the buttons closed, instead of having an open button down shirt.

 

It is much more common to see men with unbuttoned shirts. Obviously not the whole way down - but having anywhere between 1-4 buttons unbuttoned is extremely common in the sweltering Mediterranean heat.

 

The main idea behind this fashion sensation is that wearing a full suit in the height of summer is not only uncomfortable - it's old fashioned. Most modern fashion styles are much more practical than they were before. The modern man is thought about in a different way to previously. Being as formal as possible is no longer as important as being functional.

This look can be achieved by having either one, two, three - or even four buttons open on your shirt. The idea isn't to show too much skin (although you might), it's more about showing an effortless elegance without being uncomfortably hot.

 

Even dress shirts can be unbuttoned in this way - it normally works best with a more casual linen shirt, but a dress shirt is just as possible. It may seem strange to have this unbuttoned shirt style with a collared shirt - But this is also incredibly common!
